Understanding the Louisiana Occupational Therapy Licensure Process
Embarking on a career as an Occupational Therapist (OT) or Occupational Therapy Assistant (OTA) in Louisiana requires a clear understanding of the licensure process managed by the Louisiana State Board of Medical Examiners (LSBME). Whether you're a recent graduate or a seasoned professional relocating to Louisiana, this guide will help you navigate the essential steps to secure your license.
Starting Your Application
To initiate your licensure application, you must log in to the LaMED DashBoard. This portal allows you to begin a new application, continue a saved one, or check the status of a completed application. Remember, the process requires both online submissions and physical documents mailed to the LSBME.
Key Documents and Requirements
- Oath or Affirmation: This document must be notarized and any 'yes' answers need a sworn affidavit. It's crucial to be honest about your criminal history to avoid complications.
- Third Party Authorization: A notarized form allowing LSBME to obtain information from third parties.
- Certificate of Dean/Registrar: Complete Section 1 and forward it to your educational institution for completion and direct mailing to LSBME.
- Official Professional School Transcript: Must be sent directly from your school to LSBME.
- Background Check: Conducted by LSBME; instructions are available on their website.
Additional Documentation
- Birth Certificate/Valid Visa: Submit a notarized copy based on your citizenship status.
- Marriage Certificate/Name Change: If applicable, provide a notarized copy of official name change documentation.
Licensure Fees
Fees must be paid via credit/debit card and are non-refundable:
- Occupational Therapist: $150.00
- Occupational Therapy Assistant: $100.00
- Temporary License (OT): $200.00
- Temporary License (OTA): $150.00
Navigating Temporary Licensure
