Licensed Clinical Social Worker by Examination Instructions

- Legibly Completed Application Form with Original Signature: Mail the application form to the address provided. You will receive an email confirmation within 7-10 business days after your application and fee have been received.
- $393 Application Fee: This includes $39 for a background check and $4 for an NPDB check. Make your payment to the State of Wyoming. Note that all fees are non-refundable.
- Proof of Lawful Presence: Submit a completed form and a copy of a document from LIST A or copies from LIST B and C as proof of lawful presence. Ensure the name on your application matches your proof of legal presence. If your name has changed, provide a copy of the legal document that allowed for the name change (e.g., marriage certificate, divorce decree).
