Navigating the process of submitting fingerprint cards in California can be a daunting task, especially for those new to the field of online therapy services. Here at TinyEYE, we understand the importance of ensuring legal compliance while making the process as smooth as possible for our team members. This blog will provide a step-by-step guide to help you through the process of submitting fingerprint cards to the California Board of Occupational Therapy.

Why is a Background Check Required?

The California Board of Occupational Therapy requires a Department of Justice (DOJ) and Federal Bureau of Investigation (FBI) criminal history background check for all applicants. This ensures that all therapists meet the necessary legal and safety standards before providing services to students.

Fingerprint Card Procedures

Follow these steps to complete and submit your fingerprint cards:

  1. Request Fingerprint Cards: If you are unable to complete the Live Scan process in California, contact the Board to request fingerprint cards to be sent to you.
  2. Complete the Fingerprint Cards: Use BLACK INK to fill out all items on the fingerprint cards. Ensure that the name on the fingerprint cards matches the name on your application. Use the following abbreviations for physical description items:
    • Sex: Female – F, Male – M
    • Race: American Indian or Alaskan Native – I, Black – B, White – W, Asian or Pacific Islander – A, Hispanic – H, Unknown – U
    • Height (HGT): Express in feet and inches. Example: 5'9”
    • Weight (WGT): Express in pounds. Example: 140 lbs
    • Color of Eyes: Black – BLK, Blue – BLU, Brown – BRN, Gray – GRY, Green – GRN, Hazel – HZL
    • Color of Hair: Bald – BLD, Black – BLK, Brown – BRN, Blonde – BLN, Gray – GRY, Red/Auburn – RED, Sandy – SDY, White – WHI
  3. Fingerprint Rolling: Take the completed fingerprint cards to a local law enforcement agency where your fingerprints can be rolled and imprinted in BLACK INK on each card. Call ahead to ensure the service is available, if an appointment is needed, and if there is a fee.
  4. Submission: Submit your completed fingerprint cards and application to the Board in a 9” x 12” envelope. Ensure the cards are not folded by writing 'DO NOT FOLD' on the envelope and including the chip board provided. You can pay the fingerprint processing fee online at www.breeze.ca.gov or include a $49.00 check, cashier's check, or money order made payable to the California Board of Occupational Therapy or CBOT.

Note: A license will not be issued until the Board receives the background information from the DOJ. Fingerprint fees are non-refundable and subject to change without notice.
