Understanding the Application Process
To practice as an SLP in Virginia, you must complete an application process that includes several key components. Let's break it down step by step:
- Laws and Regulations: You need to attest that you have read and understood the applicable laws and regulations.
- Application Processing and Documentation: It's your responsibility to ensure that all required documents are sent directly to the board office by email, fax, or postal mail. Allow 21 business days for processing. You will receive an initial email confirming receipt and listing any missing documentation.
- Application and Fee: There is an $85.00 fee for upgrading from a VA Provisional License to a Full License. All fees are nonrefundable.
- National Practitioner Data Bank Query (NPDB): Request a current digital self-query report from the NPDB and email it to the board office.
- Initial License Expiration Dates: Licenses issued before January 1 expire on June 30 of the following year. Licenses issued after January 1 expire on June 30 of the second year.
- Board Communication: The board communicates with applicants via email.
Initial Licensure Requirements
If you held a provisional license to complete clinical hours or have never held a Virginia license before, you need to provide the following:
- ASHA Certificate of Clinical Competence: Ensure your certificate is current and unrestricted. The board staff will download the documentation of ASHA certification.
- NPDB Digital Self-Query Report: Submit this report as part of your application.
- License Verification: Provide verification of all licenses ever held, including expired ones, from other U.S. jurisdictions. The board staff will obtain online verifications where available. For jurisdictions without online systems, you must request verifications to be sent directly to the board.
- Evidence of Active Practice: If you held a license in another U.S. jurisdiction, provide evidence of active practice within the past three years. This can be verified through the Board's Employment Verification Form, company letterhead, or tax returns reflecting your occupation.
