- Form 2: Certification of Professional Education
Do not mail this form to the NYS Office of the Professions. Instead, send it to your highest degree-granting institution. Be sure to check if there is a fee involved. - Form 1: Application for Licensure
Mail this form along with the $294.00 fee to the New York State Education Department, Office of the Professions, PO Box 22063, Albany, NY 12201. Make checks payable to the New York State Education Department. Do not send cash. - Form 3: Verification of Other Professional Licensure/Certification
This form is only necessary if you currently hold or have held a license or certificate to practice any profession. Complete and send it to the appropriate licensing/certifying authority. - Form 4A: Identification of Supervisor and Setting and Form 4B: Record of Supervised Experience
These forms must be completed by your supervisor and sent to the NYS Office of the Professions. Note that these forms cannot be reviewed until your Application for Licensure has been received and approved.
