General Information and Processing Time
It's essential to understand that the Board conducts independent background checks, and the initial processing of your application can take 4-6 weeks. If there are any discrepancies or missing information, this time may increase. Once completed, the final review can take an additional 2-4 weeks.
Application Fee
The renewal fee is $20, payable by personal check, cashier’s check, or money order to the Missouri Board of Healing Arts. Remember, no credit or debit cards are accepted, and all fees are non-refundable.
Special Considerations for Armed Forces Members
If you've served in the Armed Forces or are a spouse of an active-duty member, you may qualify for additional services. Complete the Armed Forces form and provide verification of military status with your application.
Completing the Application
Section A – Missouri Tax Compliance
Ensure you are up-to-date with Missouri state income taxes. Failure to comply can lead to license suspension.
Section B – Identifying Information
Provide your full name, contact information, date of birth, social security number, and gender.
Section C – Practice Information
List your anticipated Missouri practice address. This is crucial for issuing your provisional license.
Section D – Authorized Individuals
List up to two individuals with whom the Board can discuss your application. If none, write N/A.
Section E – Examination
Indicate if you've taken the Praxis exam, as passing scores are required for renewal.
Section F – Professional Licenses History
List all states where you hold or have held a license. Request verification for each.
Section G – Personal History
Answer all questions and provide explanations for any "yes" answers with supporting documentation.
Sections H & I – Applicant’s Oath and Notarization
Sign your application in front of a notary and attach a recent photograph.
