Are you looking to reinstate your Speech-Language Pathology (SLP) license in Virginia after it has expired for more than a year? Don't worry; we've got you covered! The process may seem daunting, but with the right guidance, you'll be back to practicing in no time. Here's a step-by-step guide to help you navigate through the reinstatement process smoothly.
1. Understanding the Basics
Before you start, it's crucial to familiarize yourself with the applicable laws and regulations. You will need to attest to having read these laws as part of your application. This step ensures that you are up-to-date with the current standards and practices in Virginia.
2. Application Processing and Documentation
When applying, you are responsible for notifying the sources of the required documents to submit information directly to the board office. This can be done via email, fax, or postal mail. Here's what you need to know:
- Allow 21 business days for the board staff to receive and process your application.
- You will receive an initial email confirming receipt of your application and listing any missing documentation.
3. Application Fee
An application fee of $135.00 is required, and all fees are nonrefundable. If your license has expired, you must apply for reinstatement.
4. PRAXIS Scores
Use Board code 9347 to have your PRAXIS scores released to the Virginia Department of Health Professions Board of Audiology and Speech-Language Pathology.
5. National Practitioner Data Bank Query (NPDB)
Request a current digital self-query report from the NPDB. There is a processing fee for this service. Once you receive the digital report, email it to the board office at audbd@dhp.virginia.gov.
6. Initial License Expiration Dates
Licenses issued before January 1 of the current year will expire on June 30 of the following year. Licenses issued on or after January 1 will expire on June 30 of the second year.
7. Board Communication
The Board will communicate with you primarily via email, so keep an eye on your inbox for updates and notifications.
