Licensure Requirements Checklist
- Complete the Online Application for an Occupational Therapist License.
- Pay the application fee of $187.40 using a Visa, Mastercard, American Express, or Discover credit or debit card.
- Submit to a Criminal Background Check.
- Answer Good Moral Character Questions, providing documentation if necessary.
- Complete Human Trafficking training as required by Administrative Rule 338.1215.
- Provide your Social Security Number or complete an SSN affidavit form.
- Submit Verification of Licensure from any state or country where you hold or have held a license.
- Demonstrate English Language Proficiency.
- Complete 2 hours of Implicit Bias Training.
Additional Requirements for Licensure by Examination
- Submit official transcripts from an accredited occupational therapy educational program.
- Pass the Michigan jurisprudence examination with a score of at least 75%.
- Pass the occupational therapist certification examination administered by the National Board for Certification in Occupational Therapy (NBCOT).
Additional Requirements for Licensure by Endorsement
If you have been licensed in another state and actively engaged in the practice of occupational therapy for:
- Less than 5 years: Meet the same requirements as an examination applicant.
- More than 5 years: Pass the NBCOT and Michigan jurisprudence examinations.
Additional Requirements for Relicensure
If your license has lapsed, you must complete the following:
- Submit an online application for relicensure and pay the fee of $207.40.
- Complete 20 hours of continuing education, with at least 1 hour in pain and symptom management.
- Pass the jurisprudence examination if your license has lapsed for more than 3 years.
- Complete supervised practice experience if your license has lapsed for more than 3 years.
Renewal Requirements Checklist
- Submit an online application for renewal and pay the fee of $165.40.
- Complete 20 hours of continuing education within the 2 years preceding the application for renewal.
- Complete Human Trafficking training and Implicit Bias training.
