Application Instructions for Licensing in Indiana
Here is a comprehensive guide to help you understand the requirements and steps involved in obtaining your license.
Application
Applications may be submitted online. Ensure you have all necessary documents ready before starting the application process.
Application Fee
There is a non-refundable application fee of $150.00, payable by credit or debit card for online applications.
Criminal Background Check
A criminal background check is mandatory for all applicants. Ensure you complete this step as part of your application.
Additional Documentation
Documentation may be uploaded during the online application process, or you can log back into your account to submit additional documents using the License Update feature.
Name Change Documentation
If your name differs from any of your documents, provide legal documentation of the name change, such as a marriage certificate or divorce decree.
Positive Response Documentation
If you answer "Yes" to any disciplinary or background questions on the application, you must provide a detailed statement explaining the situation, including the violation, location, date, and disposition. Submit copies of court documents for each instance to support your statement.
Official Transcript
Submit an official transcript from your speech-language pathology or audiology school, verifying that you possess a Master's degree or its equivalent. Audiologists must possess a doctoral degree and have completed a minimum of 1,820 clock hours of supervised clinical practicum.
Direct Supervised Clinical Experience
Provide certification from your educational institution confirming that you have completed 400 hours of direct supervised clinical experience.
Clinical Fellowship
Submit certification of completion of a clinical fellowship, meeting specific requirements. This should include evidence of at least nine consecutive months of full-time employment or its equivalent.
Official ETS-Praxis Series Scores
Submit an official score report from the ETS-Praxis Series, verifying a passing score on the examination. Request that your official scores be sent directly to the Indiana Professional Licensing Agency from ETS.
Certificate of Clinical Competence from ASHA
If you have a current Certificate of Clinical Competence (CCC) from the American Speech-Language Hearing Association, you may submit it as a substitute for several requirements, including the official transcript, direct supervised clinical experience, clinical fellowship, and official ETS-Praxis Series scores. Contact ASHA to request an original letter of verification to be sent directly to the Indiana Professional Licensing Agency.
Jurisprudence Examination
All applicants must pass a written jurisprudence examination covering the Speech-Language Pathology and Audiology Statute, Administrative Rules, and Health Professions Standards of Practice. A passing score is 75%. No applicant is exempt from taking this exam.
Important Note
You must have a Speech-Language Pathology license through the Indiana Professional Licensing Agency to practice as an SLP in Indiana, including all school districts. Some school districts may also require a license through the Department of Education in addition to the Board-issued license.
