Understanding the Transition from Paediatric to Adult Care

The transition from paediatric to adult healthcare services is a critical period for children and young people (CYP), particularly those with long-term conditions (LTCs). This transition often coincides with increased rates of emergency admissions, as highlighted in the research article "Emergency admissions and long-term conditions during transition from paediatric to adult care: a cross-sectional study using Hospital Episode Statistics data."

Key Findings from the Research

The study analyzed emergency admissions in England between 2009 and 2011, focusing on CYP aged 10–24 years. It revealed that emergency admission rates increased significantly during the transition period, especially for those with LTCs. The incidence rate ratio (IRR) for CYP with LTCs was 1.55, indicating a 55% increase in emergency admissions compared to those without LTCs, who had an IRR of 1.21.

Notably, the increase in admissions was most pronounced for CYP with mental health issues, metabolic, endocrine, digestive, renal, genitourinary (MEDReG) disorders, and multiple LTCs. The study also found that the length of stay for emergency admissions increased with age, particularly for those with mental health problems.

Implications for Practitioners

For practitioners, these findings underscore the importance of targeted interventions to support CYP during this vulnerable transition period. Here are some strategies to consider:

Practitioners are encouraged to engage in or support research initiatives that focus on improving transition care, particularly for those with LTCs.
