Essential Steps for Obtaining Your NYS Licensure as a Speech Language Pathologist

Licensure Requirements

- Meet the test requirements (ETS Praxis II)
- Meet the experience requirements
- Pay the application fee ($294.00)
First Things First
Start by obtaining your original transcripts and ETS (Praxis II) test results. This process can take several weeks, so it's essential to begin as soon as possible. Additionally, download the SLP Licensing Application Packet from the NYS Office of the Professions website.
Forms, Forms, Forms
Completing the required forms accurately is vital for your application. Here is a breakdown of the forms you need to submit:
- Form 2: Certification of Professional Education - Complete this form and mail it to your highest degree-granting institution. Check with them to determine if there is a fee you must pay. Do NOT mail this form to the NYS Office of the Professions.
- Form 1: Application for Licensure - Complete this form and mail it along with the $294.00 application fee to the New York State Education Department, Office of the Professions, PO Box 22063, Albany, NY 12201. Make checks or money orders payable to the New York State Education Department. Do NOT send cash.
- Form 3: Verification of Other Professional Licensure/Certification - Complete this form ONLY if you currently hold or have held a license or certificate to practice any profession, anywhere. Send the entire form to the appropriate licensing/certifying authority for completion of Section II. Do NOT mail this form to the NYS Office of the Professions.
- Form 4A: Identification of Supervisor and Setting - Complete this form and get it to your supervisor for completion of their part. Your supervisor must send this form to the NYS Office of the Professions.
- Form 4B: Record of Supervised Experience - Complete this form and get it to your supervisor for completion of their part. Your supervisor must send this form to the NYS Office of the Professions. Note that the Office of the Professions can’t review these forms until they have received and approved your Application for Licensure.
