Step-by-Step Guide to Licensure
Before you start your application, make sure to read and follow each of the four steps below to avoid any delays in processing. Remember, the application you submit is valid for three years from the date of receipt, and your license will expire on October 31 of each odd-numbered year.Step 1: Reference Sheet
Use the REFERENCE SHEET (CHART I) to select the appropriate Profession Name, 3-digit Profession Code, Licensure Method, and Fee. Record this information in Part I (page one) of the Application for Licensure and/or Examination.Step 2: Complete the Application
Proceed with Part II (page one) and fill out all applicable information requested on all four pages of the Application for Licensure and/or Examination. Note: Indicate your undergraduate and graduate education in Part III, number 6 on the Application.Step 3: Follow Specific Instructions
Locate the specific instructions for the Licensure Method you recorded in Part I (page one) of the Application and follow those instructions only.Note: All documents in a foreign language must be accompanied by an original, notarized translation performed by a person fluent in both English and the language of the document(s). The translator must certify the accuracy of the translation.Step 4: Need Help?

In order for your application to be processed, all required supporting documentation must be submitted with the application and required fee unless otherwise directed in the instructions.- Supporting Document PHQ must be completed and submitted with each application.
- Applicant must be at least 21 years of age.
- Supporting Document ED must verify either a master’s or doctoral degree from an approved speech-language pathology or audiology program.
- If you have ever been licensed, Supporting Document CT must be completed by the U.S. jurisdiction of original licensure and the U.S. jurisdiction of current licensure where you have most recently been practicing.
- Supporting Document VE must be completed for speech-language pathologists and audiologists verifying the successful completion of 9 months of full-time, supervised professional experience.
- Submit verification of successful completion of the National Examination in Speech-Language Pathology or Audiology (PRAXIS) in the form of original grade results from the NTE Program; or for speech-language pathologists, submit a copy of certification from the American Speech-Language Hearing Association (ASHA).
- Fee payment can be made via check, money order, or online.
