Licensure Requirements Checklist
Here are the minimum requirements needed to obtain licensure:
- Complete the online application for a Speech-Language Pathologist License by visiting www.michigan.gov/miplus.
- Pay the application fee of $187.40 using Visa, Mastercard, American Express, or Discover.
- Complete a Criminal Background Check as instructed in the Application Confirmation letter.
- Answer Good Moral Character Questions and provide documentation if necessary.
- Complete Human Trafficking training.
- Provide your Social Security Number or SSN affidavit form.
- Submit Verification of Licensure from any state or country where you hold or have held a license.
- Demonstrate English Language Proficiency.
- Complete 2 hours of Implicit Bias Training within the 5 years immediately preceding the issuance of the license.
Additional Requirements for Licensure by Examination
- Provide the name of the school attended and the educational program completed.
- Submit verification of current American Speech Language Hearing Association (ASHA) certification.
- Submit final, official transcripts from your educational institution.
- If educated in a non-accredited institution, submit a full course-by-course credential evaluation.
- Complete 9 months (1,260 hours) of supervised post-graduate clinical experience.
- Pass the PRAXIS Series II Exam in Speech-Language Pathology.
Additional Requirements for Licensure by Endorsement
For those licensed in another U.S. state or a province of Canada:
- Submit verification of licensure from the state or province.
- Submit verification of current ASHA certification or final official transcripts.
- Submit PRAXIS Series II Exam scores.
- Provide proof of 9 months (1,260 hours) of supervised post-graduate clinical experience if licensed for less than 5 years.
Additional Requirements for Relicensure
- Complete the online application for relicensure by visiting www.michigan.gov/miplus.
- Pay the application fee of $207.40.
- Submit proof of 20 hours of continuing professional development courses, including 1 hour in pain and/or symptom management.
- If the license has been lapsed for more than 3 years, satisfy one of the following:
- Possess a current Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA.
- Provide proof of registration or licensure in another state during the 3-year period immediately preceding the application.
- Retake and pass the PRAXIS Series II Exam in Speech-Language Pathology.
Renewal Requirements Checklist
