Method 1: Current ASHA Certification
If you already have your American Speech Hearing Association (ASHA) certification, you're on the right track! Follow these instructions to complete your application:
- Print the provided document and use it as a checklist.
- Complete the attached application for licensing as a speech language pathologist.
- Answer every numbered question or statement in the application. Incomplete applications will be returned.
- If something does not apply to you, write "N/A†in the space provided.
- Ensure each question is answered fully, truthfully, and accurately.
- Sign the Records Waiver Authorization and Release.
- Complete, sign, and date the application forms within 30 days of submission.
- Contact ASHA to have a verification of your credential sent directly to you in a sealed envelope. Do not open the envelope.
- Mail the unopened and sealed envelope containing the ASHA verification to the office with your completed application packet and fee.
- Print, complete, and send the Verification of Credential Form (PDF) to each state where you hold or have held an SLP/AUD license.
Method 2: Reciprocity
If you hold a current and unrestricted credential as an SLP or audiologist in another jurisdiction with equivalent or higher requirements, you may qualify for licensing by reciprocity. Here's what you need to do:
- Print the provided document and use it as a checklist.
- Complete the attached application for licensing as a speech language pathologist.
- Answer every numbered question or statement in the application. Incomplete applications will be returned.
- If something does not apply to you, write "N/A†in the space provided.
- Ensure each question is answered fully, truthfully, and accurately.
- Sign the Records Waiver Authorization and Release.
- Complete, sign, and date the application forms within 30 days of submission.

Method 3: Meet All Requirements but Do Not Have Certification
If you meet all the requirements for ASHA certification but do not currently hold a CCC or board certification, you can still qualify for licensing. Follow these steps:
- Print the provided document and use it as a checklist.
- Complete the attached application for licensing as a speech language pathologist/audiologist.
- Answer every numbered question or statement in the application. Incomplete applications will be returned.
- If something does not apply to you, write "N/A†in the space provided.
- Ensure each question is answered fully, truthfully, and accurately.
- Sign the Records Waiver Authorization and Release.
- Complete, sign, and date the application forms within 30 days of submission.
- Contact the educational institution from which you received your degree and request an official transcript in a sealed/unopened envelope. Do not open the envelope.
- Contact the institution where you completed your supervised clinical training and have the appropriate person(s) sign and date Form A.
- Contact the organization where you completed your supervised post-graduate clinical fellowship or doctoral internship experience and have the appropriate person sign and date Form B.
- Print, complete, and send the Verification of Credential Form (PDF) to each state where you hold or have held an SLP/AUD license.
