Application for Licensure by Examination
Instructions for All Applicants:
- You cannot practice Occupational Therapy in Wyoming until your application has been approved by the Board.
- All application forms must be signed and must contain a current photograph.
- All applicants are required to show they are eligible for licensure by submitting either a single document from List A or a document from List B and List C.
- Applications must be mailed to the address above. The application should include the fee and the proof of lawful presence form. You must send the original of the application and not a copy.
- All communication regarding your application will be sent to the email address you provide on the application. Questions should be directed to Chris Frint at christopher.frint@wyo.gov. Information on the status of the application will only be shared with the applicant.
- Processing time, once an application is complete, is 7-10 business days.
- If additional space is needed to provide more information regarding a question on the application, attach additional sheets. Each sheet should include your full name as presented on the application.
Additional Information:
- Passing Scores of the National Exam: The Certification Examination for Occupational Therapist, Registered (OTR) as administered by the NBCOT or the current examination recognized by the board. Have your official score report sent to Wyoming.
- OTA Supervision: Per the Rules & Regulation, Chapter 3, Section 2. Supervision of Certified Occupational Therapy Assistants or Occupational Therapy Assistants. A licensed OTA may assist in the practice of occupational therapy only under the supervision of a licensed OT. There is a Supervision Log form available on the website, that the supervising OT and OTA will need to use.
- National Practitioner Data Bank Query: The Board queries the National Practitioner Data Bank when applicants submit an application in order to determine whether any other state has reported an adverse action against a licensed occupational therapist or any healthcare-related field. By submitting this application, you agree that the Board may submit these queries.

- License Verification: Required of all licenses currently held. You will need to request an official verification from any state you hold an occupational therapy or occupational therapy assistant license. Many jurisdictions charge a fee to provide written verifications, and it is the responsibility of the applicant to ascertain this as well as pay the required fee. The Board does have a verification form available, but the board will accept the information in any format provided by the jurisdictions.
