Here’s a straightforward guide to help you navigate the renewal and reinstatement process for your SLP license in Utah.

Steps for Renewal

To renew your license, you will need to fill out the renewal/reinstatement form. Here’s a breakdown of what you'll need to do:

  1. Provide your name, address, phone number, and email.
  2. Answer the qualifying questionnaire honestly. Remember, false or misleading answers can result in loss of licensure or criminal prosecution.
  3. Submit any additional required documentation if you answered "Yes" to any of the qualifying questions.
  4. Sign the affidavit certifying your eligibility and compliance with renewal requirements.
  5. Pay the renewal fee of $47.00. If your license has expired, additional fees will apply.

Continuing Professional Education

As part of the renewal requirements, you must complete 20 hours of continuing professional education related to your practice every two years. If you received your initial license during the current renewal cycle, you only need to complete a pro-rata amount of education hours.

What If Your License Has Expired?

If your license has expired, you can still reinstate it by following these steps:
