For Applicants with a Valid License in Another State or ASHA CCCs
If you already hold a valid license to practice speech-language pathology in another state or have your ASHA Certificate of Clinical Competence (CCC), follow these steps:
- Download and fill out the IDFPR Application.
- Follow the instructions for licensure by endorsement of Licensure Profession code: 146.
- Complete the CAA form (regardless of criminal background).
- Complete the top portion of the CT form and send it to the state licensing board for your initial and current license.
- Complete the top portion of the ED form and send it to your graduate school.
- Complete the VE form and send Praxis scores or a copy of your ASHA CCC.
- Mail the completed packet with returned CT and ED forms and a $100 check payable to the Illinois Department of Financial & Professional Regulation.
For Applicants Without a Valid License or ASHA CCCs (Recent Graduates)
If you do not hold a valid license in another state or do not have your ASHA CCCs, follow these steps:
- Download and fill out the IDFPR Application for both temporary and full state licenses.
- Follow the instructions for licensure by Acceptance of Examination and licensure by Temporary License (TP-SLP). Profession Code: 146.
- Ask ETS to send your Praxis scores (test code 5331) to IDFPR.
- Complete the CAA form (regardless of criminal background).
- Complete the top portion of the ED form and send it to your graduate school. (IL graduates should contact their program director.)
- Mail the VE form to IDFPR after CF completion.
- Mail the completed application, CAA form, and a $165 check payable to the Illinois Department of Financial and Professional Regulation.
